I tried the current 2025, but it won't even open. I choose the 'Portable' version if that mattered. I then tried a couple of older versions, but none will even open.
I'm running W7 Pro x64. No A/V or blocker programs running or installed.

I'm using the latest NTLite on W7. Obviously you can't do Updates or Appx integration for W10/11 images, but otherwise it works fine for other tasks. If NTLite isn't launching, check your anti-virus security's activity log.

The only other suggestion is to rename "Program Files\NTLite" to NTLite.old, and re-install. You may have some corruption in the settings file, or the cache folders which is crashing NTLite on launch.

I don't get any response when I click on the entry and Process Hacker (it's not a hack tool) shows no instance of the program running. I'll do a re-boot and see if that does anything after another (un-related) program that is running finishes.

No difference.
I took a closer look at Process Hacker. I do get a response for a second or two, the program does try to start, but closes immediately sometimes along with a MS process which I can't make out due to the briefness of the entry in the list.

Does this need/require/use an additional MS process/dependence etc to run?

NTLite doesn't have a lot of dependencies, it's deliberately coded that way. Is there a crash file under your AppData\Local\Temp folder?

I combine all the Temp folders in the root of C (environmental variables). I found no error type of file connected to that program.
How is it named?

Closest to that is;
C:\Users\Default\AppData\Local\Temp
and it's empty since all of those unnecessary Temp folders are one. I don't have to search for them when it's one click away in C drive. Makes finding viruses easier or any other annoyances that get planted from other programs installations that like to reside in one of them. ;)

I now believe it's a O/S issue, thou no other program seems to be affected.

Every new user account is cloned from the Default User's profile settings, before Windows applies other changes.

Otherwise, it's not used after that. You should be using a per-account AppData\Local\Temp, and NTLite defaults to storing files under there.

I'm in the middle of restoring a older W7 install, I'll see how that works. But, since I haven't actually 'ran' the program, it don't see why/how any other folders would be created.
